Visit Us In-Clinic or Virtually
Although we are based in Caulfield, all of our team, including the allied health practitioners and our specialised clinicians offer telehealth appointments for those who may be unable to attend the clinic in person. Meaning that you can access our specialised and outstanding care service no matter your location.
Your First Appointment
Once you have secured an appointment, you may wish to complete our Patient Registration Form prior to your appointment so that you don’t have to complete it on the day.
If you prefer to complete your questionnaire on the day in person, then please arrive earlier to allow time for your paperwork to be completed
What To Bring
- Updated referral from your GP(if requested)
- Medicare, DVA and Pension cards
- Any relevant or requested blood test results.
- A current medication list
- Your patient registration form if completed in advance
